Former Center for Mental Health Consumers Invited to Access Their Records

Former Center for Mental Health Consumers have until May 19, 2007 to request their records.

(<?xml:namespace prefix = st1 ns = "urn:schemas-microsoft-com:office:smarttags" /?>Washington, DC)  The DC Department of Mental Health wants former Center for Mental Health (CMH) consumers to learn how they can obtain their patient records. All former CMH consumers may access and request their records until May 19, 2007. After this date, the records will be destroyed.  CMH ceased operations in December 2005 and filed for bankruptcy in January 2006.  Consumers also may call the DMH Office of Consumer and Family Affairs at (202) 673-4377 for more information.

Consumers should use the patient form issued by the CMH bankruptcy trustee, Stinson Morrison Hecker, to request their records.
