The following information is to provide guidance on filing paper claims to DHCF in three (2) categories in which claims have been denied for payment:

1. Claims that were denied in eCura and the provider did not re-work and resubmit.

a. Resubmit on paper to DHCF to the attention of Ms. Marlena Edwards.

2. Claims that were denied as duplicates and the Provider can document that they are not duplicates but were not rolled-up.

a. Resubmit on paper as adjustment.  Place the Transaction Control Number (TCN) on the bottom of the Claim and an (A) for Adjustment at the top of the claim.
b. Place an adjustment reason code on the claim.
c. Resubmit on paper to the attention of Ms. Marlena Edwards.
