Welcome to the DBH Provider Certification Application Page
Thank you for your interest in becoming a certified provider with the Department of Behavioral Health (DBH). Before you begin the certification process, please carefully review the information below.
To determine which services you are eligible to apply for, please refer to the linked Moratorium Notification. This document will guide you in confirming your eligibility. You will also find a blank copy of the Application for Certification as a DBH provider, which must be completed in full.
Important Requirements and Instructions:
National Accreditation Requirement: Before submitting your application, you must obtain National Accreditation. You are required to submit either your accreditation certificate or evidence of accreditation as part of your application.
Submitting Your Application: Once completed, email your application to the Certification Division at [email protected]. To streamline the process, only submit the documents specifically requested on the application.
File Size Considerations: If your application contains large files, please divide them into smaller parts and send each part in a separate email. This will help avoid issues with email size limits.
