The Funding Alert, a free resource produced by Serve DC, highlights grant opportunities and resources for community-based, faith-based, 501 (c) (3) nonprofits, and District government agencies. Eligibility Documents

The following list of documents are required for every grant application. These documents are used to verify that an applicant organization is eligible to receive grant funds from the District of Columbia and thereby the Department of Behavioral Health (DBH).

Certificate of Clean Hands dated within 60 days of the application deadline

Current Business License/Certificate of Licensure or proof to transact business in local jurisdiction

Current Certificate of Insurance

Copy of Cyber Liability Policy

IRS Tax-Exempt Determination Letter (for nonprofits only)

IRS 990 Form from most recent tax year (for nonprofits only)

Current Fiscal Year budget OR Financial Statements

Current List of Board of Directors, on letterhead, signed and dated by a certified official from the Board (this cannot be the Executive Director)

Active Unique Entity Identifier #

Assurances and Certifications

If you are applying for a Department of Behavioral Health (DBH) grant, you must download the application forms found in the DBH Application Package and read the Request For (RFA) carefully to determine which forms you must include in your application. Applications that do not include all required forms will be screened out and will not be eligible for review.
