Community-based Intervention (CBI) Level II and III Evaluation Protocol:

CBI providers shall utilize the monthly CBI Authorization and Access to Services Report developed by DMH and distributed to CBI providers monthly. This report is designed to be utilized as a quality assurance tool, which monitors timely access to service through the tracking of all CBI referrals from authorization request through the first service for the month prior. 

CBI providers shall utilize the monthly CBI Service Utilization Variance Report developed by DMH and distributed to CBI providers monthly. This report is intended to serve as a decision support tool for CBI program managers and providers to assist in monitoring and improving appropriateness and access to care for child and youth consumers who meet eligibility for CBI services.

CBI providers shall utilize the attached Intensive Home-Based Treatment (IHBT) Fidelity monitoring tool [PDF] for periodic internal quality assessment. The measures in this tool will be utilized by DMH to conduct quality review beginning in FY13.
