Effective as of the above date, the following signatures are the only ones acceptable as the Approving Qualified Practitioner on the IRP/IPC:

• Psychiatrist
• Psychologist
• Licensed Clinical Social Worker (LICSW)
• Advance Practice Registered Nurse (APRN)
