Welcome to DBH’s Incident Reporting Page
The Department of Behavioral Health (DBH) investigates reports of Major Unusual Incidents (MUI) and certain Unusual Incidents (UI). DBH tracks MUI/UI data, writes investigative reports, and submits findings to the agency Director, General Counsel, and other appropriate parties.
An MUI/UI must be completed and submitted by the first person who learned/witnessed/discovered an MUI/UI per DBH Policy 480.1.
DBH Incident Categories & Definitions
Serious incidents must be verbally reported to DBH immediately, no later than one (1) hour after discovery/learning of the incident (i.e., unexpected death, incident requiring law enforcement or emergency medical services, or incidents related to consumer care that raise immediate concerns from the determination of the provider agency regarding the health and safety of any consumer, employee, or visitor). Written MUIs/UIs must be submitted within 24 hours per DBH Policy 480.1.

- Outside of normal business hours (before 8:30 a.m. or after 5:00 p.m., Monday through Friday, weekends and holidays), contact the Compliance Hotline at (800) 345-5564.
